Follow these instructions to verify your employer sponsored health insurance:

Call the number on your card to verify whether your behavioral health/mental health benefits include telehealth visits when the patient is at home. (Some plans cover telehealth visits only when the patient comes to the office and does a virtual visit with a provider who is located in another office, so you need to clarify that you will be staying at home for this visit.)

If your health plan does NOT cover the visit, you can provide Hope Health with updated proof of income to see if you qualify for the sliding scale.  If your household income is less than 200% of the 2022 Federal Poverty Guidelines, the cost for the session will be between $10-$25, depending on your exact income. If your household income is over that amount, the full cost depends on the service and ranges from $65-$253.
