“WHAT IF I DON’T HAVE INSURANCE?”

As a Federally Qualified Health Center, we provide care to anyone, regardless of income, residency status, employment, or Health Insurance coverage. If you are uninsured, fees for service will be based on a sliding fee scale determined by your household income and family size. If you qualify, you could receive up to an 85% discount.

Hope Health services are not free. We request payment in the form of a copay at the time of visit. If you are unable to pay for full services at the time of your visit, a payment plan is presented with a 20% down payment. Hope Health accepts payment in the form of cash, check, credit card or money order. If you need to mail in your payment, you can do so too:

“COULD I BE ELIGIBLE FOR HEALTH CARE BENEFITS OR OTHER COMMUNITY SERVICES?”

Make an appointment with one of our Enrollment Specialist to see if you may qualify for any health care programs such as Medicaid, CHIP, SNAP, TANF, or an Affordable Care Act (Marketplace) plan. We can assist in determining which programs you may qualify for and provide assistance in completing the appropriate forms. Our Enrollment Specialists are certified in both Marketplace and Medicaid programs. This is a FREE service and you do not have to be a Hope Health patient to receive assistance.

We accept most health insurance plans, including major carriers like Blue Cross Blue Shield, UnitedHealthcare, Aetna, Cigna, and many others. To verify we accept your insurance plan, call us at 972-923-2440.
